The everyday problem

A meeting that reads out denial counts can end without changing anyone’s work. The count describes the outcome but does not explain whether the issue began with information capture, a handoff, or a follow-up process. Teams need a path from the report to an operational decision.

A practical approach

Select a small number of representative examples and trace how each reached the current state. Distinguish a recurring process issue from an isolated exception. Assign a specific change, a responsible person, and a review date rather than asking every department to improve generally.

Create a meeting agenda with four columns: example, contributing handoff, proposed action, and owner. Revisit the previous meeting’s actions before adding new ones.
